Home Care Support
The Home Care Support Services provides care and support to whānau to enable them to live independently within their own homes. To be eligible you need to be aged 65 years or older, or 50-64 with age related needs, or requiring long term support for chronic health conditions.
Entry to this service is via referrals from Support Net or Community Care Coordination service at the hospital. Your doctor or nurse can also refer to Support Net for an assessment.

- We support with Home Help Assistance (meal preparation, cleaning, laundry etc)
- Personal Cares (washing, dressing etc)
- Postoperative care and mobility exercises
- Carer support and respite care
