Supporting Tamariki/Rangatahi Wellbeing
At Pirirākau Hauora we offer compassionate and culturally grounded support for tamariki/rangatahi aged 0-19 years and up to their 20th birthday, experiencing low to medium mental health needs. Our services are tailored to meet the unique circumstances of each individual and whānau, ensuring everyone feels seen, heard, and supported on their journey to well-being.
What We Offer:
🌿 Mental Health Support: Personalised support for individuals and whānau with low to medium mental health challenges. We provide a safe and understanding space to help navigate life’s difficulties.
📝 Holistic Assessments: We work alongside tamariki/rangatahi and their whānau to gain a full picture of their needs, strengths, and aspirations.
📄 Tūāpapa Care Plan: Our Tūāpapa System of Care respects each person’s right to choose what is right for them based on their own beliefs and customs. It allows different approaches to be used to meet the needs of whānau and considers all things that can lead to good health and wellbeing.
A collaborative care plan is developed to guide and support each individual/whānau with practical steps toward their wellbeing goals, using the Tūāpapa System of Care. This unique model, created by our kaumātua, is grounded in the values of tika (doing what is right), pono (integrity and truth), and aroha (compassion and love).
🔄 Follow-up Visits: Ongoing check-ins to ensure consistent support, to build trust, and adapt plans as needed.
🤝 Overall Support for Your Needs: Whether it’s connecting with services, providing emotional support, or helping navigate systems, we’re here to walk alongside you every step of the way.
