Whānau Ora Kaiārahi

The Whānau Ora Kaiārahi supports whānau to reach their whānau aspirations and wellbeing through goal planning and connections. The aim is to ensure whānau receive the services they need, enable them to assess their current position and determine their future goals. Planning for future success starts with whānau and their aspirations are determined by their actions to meet their goals.

  • Create a plan and steps to achieve the desired outcome
  • Access resources
  • Help whānau overcome barriers that may impact on achieving their goals
  • Referring to the right services whether it is for health, education or other needs
  • Advocacy with MSD, OT, ACC
  • Refer and support whānau to achieve their goals and gain better health outcomes
  • Connect whānau to appropriate organisations
